• Diploma in Pharmacy from an institute recognized by the Pharmacy Council of India.
• Registered with the State Pharmacy Council of Uttar Pradesh.

2. Complete the application form with educational details and upload required documents.
3. Pay the required application fee (Rs. 25) through the SBI m-collect gateway.
4. Check all details carefully before final submission and take a printout of the application form.
